چکیده

A 69-year-old woman with hypertension was admitted for an acute coronary syndrome. Emergency coronary angiography showed a long, bifurcated, calcified lesion in the left anterior descending artery (LAD). An interventional procedure was performed by predilations with an expandable and a nonexpandable balloon. Following the last inflation, angiography showed LAD occlusion due to a perforation, which resulted in an arteriovenous fistula toward the anterior interventricular vein (Figures A-D, arrows: sequential images showing passage of contrast from the LAD to the anterior interventricular vein). Guide wires placed in the LAD and diagonal branch [video 1] caused the perforation leading to cardiac arrest due to ventricular fibrillation, from which the patient recovered after 2 defibrillation shocks and cardiac massage. Prolonged balloon inflation was performed in the area of the rupture and 50 mg of protamine was administered. After confirming that the perforation had sealed, 4 drug-eluting stents were implanted in the proximal-medial LAD. Following implantation of the last stent, the patient became clinically unstable, coincident with a diffuse ST-segment elevation in the precordial leads, secondary to extensive stent thrombosis (video 2). This was successfully resolved with reheparinization, intracoronary abciximab and balloon inflations. The patient has remained asymptomatic after more than 1 year of follow-up. Coronary perforations tend to drain into the pericardium and only exceptionally into the cardiac chambers (Ellis type III cavity spilling). Coronary arteriovenous fistulas secondary to a coronary perforation are rare, with only 2 reported cases. Protamine use is controversial, although supported by experts, and it produced stent thrombosis in our patient. Deferred revascularization or the use of coated stents to seal the coronary perforation would have been valid alternatives to avoid protamine use and the resulting risk of stent thrombosis.

Coronary Stent Embolisation to an Iatrogenic Arteriovenous Fistula: An Unexpected Complication

INTRODUCTION Systemic embolisation of stents represents a rare complication of coronary interventions. REPORT The case of a patient with peripheral embolisation of a coronary stent while undergoing a coronary procedure for an ischaemic event is reported. The stent lodged at a previously created iatrogenic fistula joining the profunda femoral artery and the common femoral vein. An endovascular...

Arteriovenous fistula as a complication of transradial coronary angiography: a case report

UNLABELLED INTRODUCTION Iatrogenic arteriovenous fistula is a vascular condition that may result from coronary angiography. Many case reports have described arteriovenous fistula occurrence after coronary angiography using the transfemoral access route, but rarely as a complication of using the transradial approach. We report a rare case of a patient with arteriovenous fistula following tran...

Huge arteriovenous fistula between a giant aneurismal right coronary artery and coronary sinus.

Coronary arteriovenous fistulas are rare and may be congenital, traumatic, or iatrogenic. We report one of these rare congenital anomalies with a giant aneurismal right coronary artery (RCA) in a middle-aged lady who was asymptomatic for a long time.

Rupture of the Coronary Sinus with Hemopericardium. a Rare Complication of Coronary Arteriovenous Fistula.

MANY excellent reviews of coronary arteriovenous fistulas have recently appeared in the literature.1 2 In the majority of these fatal cases, death was attributed to congestive heart failure. Rupture of the eoronary sinus as a coinplication of a coronary arteriovenous fistula has not, to our knowledge, been reported. The present case is thought to be unique in that the fistula was accompanied by...

Coil embolization of iatrogenic coronary-pulmonary arterial fistula after heart transplantation

Coronary arteriovenous fistula (CAVF) is defined as direct connections between the coronary artery and one of cardiac cavities. It is generally derived from right coronary artery (RCA). Fistulas may be congenital or acquired (1). Coronary-pulmonary arterial fistulas (CPAF) constitute 15%-20% of all fistulas. They are mostly small, determined co-incidentally during coronary angiography (CAG), an...
